Continuum-of-Care for Older Adults

Continuum-of-care facilities have become increasingly popular over the years due to the prevalence of patient-centered care. Continuum-of-care is described as a patient-centered system that can span over several years of a patient’s life, including elderly and end-of-life care. It guides patients through comprehensive health services spanning all levels and intensity of care. Continuum-of-care facilities are designed to provide several levels of services that meet the needs of an individual. These services may include independent living, assisted living, memory care, and nursing home services. A continuum-of-care facility aims to allow individuals the ability to live comfortably in their homes for as long as possible. 

With patient-centered care as the primary goal, each continuum-of-care facility differs in the services they provide. Sometimes, continuum-of-care services might only be needed for a limited time while a patient recovers from an acute illness or injury. In contrast, others can utilize services for chronic diseases. Some additional care options may include personal care, transportation, rehabilitative services, adult daycare services, long-term care, and hospice.

There are many benefits to continuum-of-care services. As previously mentioned, individuals can stay in their homes longer without the stress of relocating. The trauma of relocating for older adults may cause unnecessary worry that can negatively affect their health, such as depression, loss of sleep, and anxiety. Another benefit is that services can be adjusted to meet an individual’s needs. These services allow for more personalized care that can change through different stages of life. Continuum-of-care services aim to support individuals’ mental health and well-being as they navigate through life.

Planning for the future is critical to safeguard wellness. Continuum-of-care services are available for just that. After all, growing older should be enjoyed without worrying about complex decisions. Some retirement communities may offer continuum-of-care services to reduce the headache of decision-making. Older adults may decide to move into an assisted living community that provides continuum-of-care services to prepare for the future and allow for a more seamless transition because retirement is meant for fun, not worry!
