What is the PACE program?

The PACE program is a great resource for seniors needing extra assistance with their personal and health care needs. The PACE program is provided by Medicare and Medicaid and strives to make necessary health care needs accessible and convenient for benefiters. These include such services as: medication, health care provider visits, transportation, home care, and more. The Welbe Health team caters to an individual and can help with around the clock care. Along with the health care attention given, social components such as providing access to games and socialization with caregivers can be very beneficial. The program not only assists to provide quality health care, but also enhances quality of life, giving seniors independence. 

Who can apply for the PACE Program?

According to Medicare, those who qualify for PACE program must:

  • Be 55 years or older 
  • Live in a service area of the PACE organization
  • Be certified by the state in regards to the need for nursing home level of care
  • Be able to live safely in the PACE community

The program does require one to live in the service area where it is provided. To determine whether or not you live within the area, your zip code may be used to locate the program on PACEfinder.

Cost and Beneficiaries 

CalPace provides a chart explaining the cost accordingly based on prior memberships with Medi-Cal and Medicare.
